  1. The teacher(s) of the student must provide class work to the homebound teacher during the time on home/hospital services.
  2. The teacher(s) must give the grade for the class work completed during the time on home/hospital services.
  3. If there are modifications or special provisions during the time on home/hospital, there must be a summary sent to the school. If there is a need, there will be a meeting with the following people: nurse(s), Director of Pupil Personnel, home/hospital teacher, teacher(s), other related school staff, and parent(s).
  4. If there are modifications or special provisions where the child comes to school on a flexible schedule, the homebound teacher must still offer the required hours for home/hospital services.
